FINANCIAL POLICY
Our office Financial Policy:
Payment is due at the time of service.
The following methods of payment are accepted: cash, check, Mastercard, American Express, Discover, Visa, and CareCredit.
Patients/guardians will be charged $40.00 for each hour of their missed appointment(s) without a 48 hour advance notice to cancel.
Patients/guardians are responsible for balances in full after 60 days if insurance has not paid.
A service fee of $40.00 will be assessed for all returned checks.
The patient/guardian is responsible for all reasonable costs and attorney fees incurred by Governor’s Park Dental Group, if payment is not received.
